How To Treat Acne?

Zits, spots, pimples…call them what you will, they are the enemy of every teenager.

Eight out of 10 adolescents experience some degree of acne. Trying every antiseptic soap and lotion available, many spend hours squishing out blackheads and looking with horror on every new eruption.

However, teenagers don’t have a monopoly on acne. To their dismay many adults develop it for the first time in their 20s or 30s. Worse again, just when they thought they had left the ‘pimply youth’ stage behind them, many see acne breaking out again in their maturity.

Acne is mostly an inflammatory response to lesions inflicted to some of the cells lining the sebum ducts caused by sebum outbreaks or sebum hardenning inside the hair follicles, and comes before the proliferation of acne bacteria within blocked pores.

Skin consists of two thin layers — an outer layer called the epidermis and a deeper layer called the dermis. The dermis contains blood vessels and fibres which give the skin its elasticity.

The epidermis contains a hard protein called keratin, which helps protect the skin. Hairs are present everywhere on the skin’s surface apart from the soles of the feet, the palms of the hands and parts of the genitals. Each hair has a root and a shaft. Small glands called sebaceous glands near the root secrete a fatty substance called sebum, which also gives the skin a protective covering. It is this area of hair and sebaceous glands that is affected when acne develops. And the substances keratin and sebum are major players in the development of acne. Bacteria at the site of a blocked gland aggravates the condition.

Acne lesions show up in areas of the skin that have a lot of sebaceous glands, such as the face, centre of the chest, the back above the waistline, shoulders and the neck.

One primary cause of acne lesions is though to be that there is too much keratin produced at the exit of the hair shaft. This blocks the surface and doesn’t allow the sebum out. Too much sebum is also being produced. Consequently the sebum builds up underneath and a plug of keratin and sebum then forms. If the upper portion of this plug is at the surface it becomes dark in colour from pigment (melanin) produced by surrounding cells in the epidermis. The result is a blackhead. If the plug remains under the surface it is referred to as a whitehead.

Sebum continues to be produced under a blackhead or whitehead and because it has no means of escape it injures the cell lining the walls of the sebum canals and then they begin to swell and the sebum and pus eventually bursts into the dermis underneath.

Redness and inflammation then occur as your body's response to injury, and pus is formed. Occasionally, in severe cases the dermis reacts by trying to ‘wall off’ the sebum and acne cysts are formed during this process.

Acne treatment must address the repair of skin lesions first and also inflammation, blocked pores, and acne bacteria, and always aim to avert acne scarring.

Healing skin lesions is a complex and dynamic process that results in the restoration of anatomical continuity and function.

There are four basic responses that can occur following a lesion:

Normal repair (reestablished equilibrium), is the response where there is a re-established equilibrium between scar formation and scar remodeling.

Excessive healing (fibrosis and contractures), there is too much deposition of connective tissue that results in altered structure and thus loss of function.

Deficient healing (chronic ulcers) is the opposite of fibrosis; it exists when there is insufficient deposition of connective tissue matrix and the tissue is weakened and fragile to the point where it can bruise and bleed easily.

Reaching the fibroblasts

The point is enzymes help the peptides and glyco messengers to reach the fibroblasts. Skin is not fragile but admirably tough, more a barrier than a membrane. That's why the outer (and toughest) layer, the stratum corneum, is also called the horny layer. It's about ten microns thick -a lot in the dermatology world- and consists of layers of the same dead cells (corneocytes) that compose fingernails. The horny layer is often compared to a brick-and-mortar construction: The large cells of the epidermis are packed tightly together, while the small spaces between them are filled with fats (lipids). The design is foolproof: The corneocytes keep foreign material out, and the lipids make sure water stays in. The second layer (the epidermis) is about 80µm micrometres or microns thick. The third layer (the dermis) contains the capillaries and nerve endings and is several millimeters thick.

A micrometre is one-millionth of a metre - one-thousandth of a millimetre. One micron is about a tenth of the diameter of a droplet of mist or fog. It can be written in scientific notation as 1×10−6 m, meaning 1 / 1,000,000 m.

Yet it's on this hard ground that most products are meant to work. Most skin-care products focus on moisturizing the 'dead' layer of the skin, the 10 microns or so that require water so they don't feel dry, but that is not enough and in most cases it is deleterious for it ends wetting the skin too much.

The combination of tough stratum corneum layer and live epidermis makes it extremely difficult for external substances to penetrate skin.
